Terms of agreement for tutoring

Please complete the form below before the first tutoring session, acknowledging the terms of agreement for working with Pen and Pause. Terms are as follows: 

Rate is determined by Pen and Pause, based on the scope of the work to be performed.  Pen and Pause will indicate the price in email or over phone after initial contact, before first session has been planned. 

Payment is due by the start of each session (for example, if a session is from 2-3pm on a Tuesday, payment is due by 2pm on Tuesday). 

Venmo and PayPal are accepted. Personal checks received  in advance of session are accepted.  

If cancellation occurs within 12 hours of booked session, full fee is due.
